The average decline

Testosterone generally declines gradually with age. Cleveland Clinic puts the average drop at about 1% a year after 30, and the long-running Baltimore Longitudinal Study of Aging found the decline in healthy men independent of illness. It also found higher body mass index went with lower total testosterone.

What a diagnosis takes

The American Urological Association uses a total testosterone below 300 ng/dL as a reasonable cut-off, measured on two separate early mornings, together with symptoms or signs. An age, or a single number, is never enough on its own.

What the labs look at

A baseline panel usually covers total and free testosterone, blood counts, a metabolic panel, PSA and estradiol, so a provider sees the whole picture before deciding anything.
