HIPAA Notice

- Your health information is protected by federal law when you get care through Freedom RX.
- It is used to treat you, bill you, and run your care. It is never sold and never used for advertising.

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information, when you receive care through Freedom RX. Please review it carefully.
1. How HIPAA applies to your care
The Health Insurance Portability and Accountability Act (HIPAA) is the federal law that protects your health information. It applies to “covered entities,” which are the healthcare providers and pharmacies who treat you, and to companies like Freedom RX that handle health information on their behalf as “business associates.”
When you complete a visit through Freedom RX, your care is delivered by independent, US-licensed providers and licensed pharmacies. They are the covered entities responsible for your protected health information (PHI), and each issues its own formal Notice of Privacy Practices, available in the patient portal. Where Freedom RX handles your PHI on their behalf, we do so under business associate agreements that legally bind us to HIPAA’s privacy and security requirements. This page summarizes, in plain English, how those protections work across your whole experience.
2. What information is protected
PHI is health information that identifies you, including your intake answers, medical history, conditions, prescriptions, treatment records, messages with your provider, and billing records tied to your care. Whether it lives in the portal, in a pharmacy system, or in an email to support, it gets the same protection.
3. How your health information may be used and disclosed
Your PHI may be used and disclosed, without additional authorization from you, only for:
- Treatment. So your provider can evaluate you, prescribe when appropriate, and coordinate with the pharmacy that fills and ships your medication.
- Payment. To bill you and process payments for your care.
- Healthcare operations. To run and improve the service, such as quality review, safety monitoring, and customer support related to your care.
- As required by law. For example, responding to a court order, mandatory public health reporting, or preventing a serious threat to health or safety.
Anyone who touches your PHI in these ways, including our service providers, is bound by contract to protect it and to use it only for the purpose it was shared.
4. Uses that require your written authorization
Almost anything beyond treatment, payment, and operations requires your written authorization first, and these in particular:
- Marketing. Your PHI is never used to target advertising and is never given to ad platforms. See our Privacy Policy.
- Sale of your information. Your PHI is never sold. We would not ask for this authorization, and HIPAA would require it if anyone tried.
- Most other disclosures not described in this notice.
If you ever sign an authorization, you can revoke it in writing at any time, except to the extent someone has already relied on it.
5. Your rights over your health information
HIPAA gives you specific rights. To exercise any of them, email support@freedomrxhealth.com or use the patient portal, and we will route your request to the right provider or pharmacy and help see it through.
- Access and copies. You can see and get a copy of your medical and billing records, usually within 30 days, in electronic form if you want it that way. A reasonable cost-based fee may apply to copies.
- Amendment. You can ask to correct records you believe are wrong or incomplete. If the request is declined, you can have your disagreement added to the record.
- Accounting of disclosures. You can ask for a list of certain disclosures of your PHI made in the prior six years, other than for treatment, payment, and operations.
- Restrictions. You can ask to restrict how your PHI is used or shared. Providers are not always required to agree, but one restriction is guaranteed: if you pay for a service in full out of pocket, which is how Freedom RX works, you can require that it not be disclosed to a health plan.
- Confidential communications. You can ask to be contacted in a specific way or at a specific location, and reasonable requests will be honored.
- Paper copy. You can ask for a paper copy of this notice or of a provider’s or pharmacy’s full notice at any time.
- Breach notification. You will be notified as required by law if a breach compromises the privacy or security of your PHI.
6. Complaints
If you believe your privacy rights have been violated, you can complain to us at support@freedomrxhealth.com, to the provider or pharmacy involved, and to the US Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/complaints. You will never face retaliation for filing a complaint. Not from us, not from your provider, not from anyone in our network.
7. Where HIPAA ends, our commitments do not
Some information you share with us, like browsing our public marketing pages, falls outside HIPAA’s technical scope. It does not fall outside our standards. As our Privacy Policy spells out, we never sell your personal information, we never share it with third parties for their own marketing, and health information is never used for advertising, HIPAA-covered or not. Any advertising cookies on our marketing pages run only with your opt-in consent, and our California notice explains how to opt out.
8. Changes to this notice
This notice may be updated from time to time, and updates apply to PHI we already hold as well as new information. The current version will always be posted here with its effective date, and material changes will be flagged more prominently on the Site or in the portal.
